PLANO BOXING GYM MEMBER AGREEMENT & LIABILITY WAIVER

By signing this document, I acknowledge that I have read, understood, and agree to the following:

Health Statement

I certify that I am physically able to participate in boxing and fitness activities. I understand that it is my responsibility to consult a physician before participating if I have any medical concerns. I agree to notify Plano Boxing Gym of any medical condition, injury, illness, or physical limitation that may affect my participation.

Photo & Video Permission

I authorize Plano Boxing Gym to photograph and/or record me during classes, training sessions, and gym events. I understand that these photos and videos may be used for the gym's website, social media, advertising, and other promotional materials without compensation.

If I do not wish to be photographed or recorded, I will notify Plano Boxing Gym in writing.

Liability Waiver

I understand that boxing, fitness training, heavy bag work, mitt work, partner drills, conditioning exercises, and sparring involve inherent risks, including but not limited to cuts, bruises, sprains, fractures, concussion, serious injury, permanent disability, or death.

I voluntarily choose to participate in these activities and knowingly assume all risks associated with my participation.

I release and forever discharge Plano Boxing Gym, its owners, coaches, instructors, employees, volunteers, and affiliates from any and all claims, demands, liabilities, damages, or causes of action arising from my participation, except where prohibited by applicable law.

I understand that sparring is optional and only permitted with coach approval.

Member Agreement

I agree to follow all gym rules and coach instructions at all times. I will treat coaches, staff, and fellow members with respect and use all equipment safely and responsibly.

I understand that unsafe behavior, misconduct, or failure to follow gym policies may result in suspension or termination of my membership.

Acknowledgment

By signing below, I acknowledge that I have carefully read this Member Agreement and Liability Waiver, fully understand its contents, and voluntarily agree to all of its terms and conditions. I understand that by signing this document, I am waiving certain legal rights.
